Mildred R. Sabo, M.D.

Dr. Mildred Sabo, M.D. is an Internist and fellowship-trained Geriatrician who has been practicing medicine for 30 years. Dr. Sabo was awarded her undergraduate degree from Rutgers University and graduated with her Medical Doctor degree from New Jersey Medical School in Newark, New Jersey. In addition, she is two-year fellowship trained in Geriatrics from New York University Hospital. 


Dr. Sabo is an expert in geriatric medicine. She was Medical Director of the Geriatric Health Center and the Memory Disorders Institute at Monmouth Medical Center. Dr. Sabo has also served as Medical Director of several senior living communities. Prior to joining the team at the Day Medical Center, Dr. Sabo was in private practice in Brick, New Jersey for 12 years.


Dr. Sabo combines research-based medicine with caring, compassionate and thoughtful patient care. She understands the unique needs of her senior patients and their families and is deeply invested in helping them maintain their optimum health and wellness.

Katie Diamond, ANP-BC, CDE

Katie Diamond earned her Bachelor of Science and Masters of Science degrees in Nursing from the University of Florida’s College of Nursing.  She is licensed and certified as an Adult Nurse Practitioner and holds certifications in geriatric nursing, oncology nursing and is a Certified Diabetes Educator. She has been a member of the Day Medical Center team for three years.   

Katie’s professional experience includes work as a Nurse Practitioner in a primary care practice in Wellington, FL. Her R.N. experience includes work in medical oncology and infusion at Bethesda Hospital in Boynton Beach, FL and the Malcolm Randall VA Medical Center in Gainesville, FL.

Katie is President of the Nurse Practitioner Council of Palm Beach County. She is passionate about geriatric medicine and diabetes care and treatment. In her free time, she enjoys attending Florida Gator football games with her husband and son and spending time at the beach.

Danae Clark Dowd, MSN, ARNP, AGNP-C

Danae Clark Dowd earned her Bachelor of Science and Master of Science degrees in Nursing from the University of South Florida.  She is licensed and certified as an Advanced Registered Nurse Practitioner, specializing in primary care for adults and geriatrics.

​Danae joined the team at the Day Medical Center with prior experience in house calls as a nurse practitioner and also several years in a local hospital as an oncology nurse. Danae is a member of the American Association of Nurse Practitioners as well as the Treasure Coast Nurse Practitioners. 

When she is not working, Danae enjoys spending time outdoors with her family.​

Meredith Cronenwett, MSN, APRN, AGPCNP-BC

Meredith Cronenwett is a Certified Adult-Gerontological Primary Care Nurse Practitioner. She has over 10 years of comprehensive healthcare experience with a specialization in gerontological care. Meredith earned her Bachelor of Science in Nursing and her Masters in Nursing from Florida Atlantic University.


Most recently, Meredith was a primary care provider in Southern California, providing on-site care to residents of skilled nursing facilities, assisted living facilities and memory care residences. Prior to transitioning to primary care, Meredith had extensive experience in acute-care settings in South Florida.

Meredith's approach to care focuses on maximizing patient satisfaction through open communication and meeting both physical and psychosocial needs. She forms a partnership with patients and their families to develop personalized, comprehensive care plans.

Meredith is the House Calls provider for the Day Medical Center.

Melissa Gonzalez, DNP, APRN, FNP-BC

Melissa Gonzalez is a Certified Family Nurse Practitioner with over 8 years of experience in healthcare practices managing acute and chronic illnesses. She holds a Doctorate in Nursing from the University of Florida.

Melissa joins the Day Medical Center after several years as the on-campus healthcare provider at Palm Beach Atlantic University in West Palm Beach. 


Melissa is committed to patient advocacy and compassionate, holistic healthcare. She is highly experienced at assessing, diagnosing and developing individualized care plans for patients of all ages. Melissa, who holds a National Council for Behavioral Health Mental Health First Aid Certification, understands the importance of incorporating wellness activities and mental healthcare into managing her patients' physical health.
